    대한심폐소생협회는 우리나라에서 발생하는 심장정지 환자들의 생존율을 높이기 위한 목적으로 2005년부터 의료인 대상 교육, 2009년도부터는 일반인을 위한 심폐소생술 교육을 하고 있습니다. 대한심폐소생협회는 일반인이 고품질의 심폐소생술을 잘 배울 수 있도록 심폐소생술 지침에 맞는 교육 동영상을 개발하였고, 2020년에는 약 47,000여명의 일반인이 전국의 220여개 BLS Training Site와 32개의 일반인 심폐소생술 교육기관에서 교육을 받았습니다. 우리나라 병원밖 심장정지환자의 발생은 인구 10만명 당 38.9명으로 보고되고 있으며, 한 해 심장정지로 인한 사망자는 29,000여명에 이르고 있습니다. 심장정지 환자가 발생했을 때 목격 또는 발견한 일반인에 의한 심폐소생술 시행 여부가 생존율에 큰 영향을 미치기 때문에 일반인을 대상으로 한 심폐소생술 교육은 매우 중요합니다. 하지만 현재 일반인에 의한 심폐소생술 시행률은 매우 낮으며 이로 인해 우리나라 심장정지 환자의 생존율도 저조한 실정입니다. 그러므로 일반인의 심폐소생술 시행률을 높이기 위해서는 이론 중심이 아닌 직접 실습을 하는 교육을 통해 실제 상황이 발생했을 때 머뭇거리지 않고 심폐소생술을 할 수 있도록 교육해야 합니다. 이에 대한심폐소생협회는 일반인들이 심폐소생술 교육용 동영상과 함께 쉽게 배우고 이해할 수 있도록 2010년 첫 번째 교재를 개발한 후, 2016년 개정판에 이어 2020년 새로운 지침에 맞춰 교재를 개정 발간하게 되었습니다. 또한, 일반인을 위한 2020년 한국 심폐소생술 지침도 발표하여 이 교재와 함께 이용함으로써 교육을 진행하는 강사들도 원활한 교육과정이 이루어지도록 하였습니다. 교재 개발을 위해 참여해 주신 BLS 위원들께 깊은 감사를 드립니다.

    If you are a combatant wounded on the battlefield, the most critical phase of your care is the period from the time of injury until the time that you arrive at a surgically capable medical treatment facility (MTF). Almost 90% of American service men and women who die from combat wounds do so before they arrive at an MTF, thus highlighting the importance of the battlefield trauma care that is provided by our combat medics, corpsmen, and pararescuemen (PJs), as well as by the casualties themselves and their fellow combatants. Most of the U.S. military went to war in Afghanistan and Iraq with battlefield trauma care strategies that were not based on Tactical Combat Casualty Care (TCCC)—medics in 2001 had no tourniquets, no hemostatic dressings, no intraosseous devices, and Civil War–vintage analgesia (IM morphine) and treated hemorrhagic shock with large-volume crystalloid fluid replacement. Only a select few Special Operations and conventional units went to war with a robust TCCC capability. The 75th Ranger Regiment, for example, taught the concepts of TCCC to everyone in the Regiment as part of their TCCC-based Casualty Response Plan. As a result, the Regiment documented a preventable prehospital death rate of 0%, an unprecedented success in optimizing casualty survival on the battlefield. In contrast, the overall incidence of preventable prehospital deaths among U.S. combat fatalities was 24%, as documented by Col. Brian Eastridge in his landmark 2012 paper.

    보건의료기관의 질 평가가 강조되고 있는 근래에는 보건의료인에 대한 체계적인 기본소생술 교육의 중요성이 지속적으로 증가되고 있다. 이에 대한심폐소생협회는 2005년 미국심장협회(American Heart Association)와 교육계약을 체결한 이래로 현재까지 보건의료인 대상으로 미국심장협회의 기본소생술 교육을 시행해 오고 있다. 미국심장협회의 기본소생술 교육과정은 동영상을 보고 따라하는 방식으로 구성되어 있기 때문에 기본소생술에 대한 체계적인 실습이 가능하며, 더불어 유기적인 강사 및 교육생 관리가 함께 시행됨에 따라 현재에는 우리나라 보건의료인의 필수교육 중의 하나로 인식되고 있다. 실제로 현재는 대한심폐소생협회 산하 220여 개의 기본소생술 교육센터(BLS training site)에서 매년 약 3,000회 이상의 기본소생술 교육이 시행되고 있으며, 매년 약 38,000여 명의 보건의료인들이 교육을 이수하고 있다. 또한 2018년에는 국내 보건의료 환경에 부합되는 대한심폐소생협회의 기본소생술 교육과정(Korean BLS course)이 개발되었으며, 이후 매년 약 450회의 교육이 약 7,500여 명의 보건의료인을 대상으로 실시되고 있다. 실제로 대한심폐소생협회의 기본소생술 교육과정은 병원밖 심장정지 상황과 함께 의료기관 내에서 발생되는 심장정지 상황을 함께 고려하고 있으며, 국내 의료기관의 현실에 발맞추어 프로그램이 구성되어 있기 때문에 보건의료 현장에서 즉시 활용될 수 있다. 향후 이 교육과정이 우리나라 보건의료인의 기본소생술 교육을 대표하는 프로그램으로 발전할 것을 믿어 의심치 않으며, 이 교재가 보다 충실한 교육이 실시되는 데 큰 역할을 할 것으로 기대한다. 이 교재는 2020년 개정된 한국심폐소생술 가이드라인을 토대로 개정되었으며, 동영상에 따라 진행되는 대한심폐소생협회 기본소생술 교육과정에서 보충해야 될 학술적인 내용과 술기평가 및 필기평가에 도움이 되는 내용을 함께 기술하였다. 대한심폐소생협회 기본소생술 교육과정 교재를 개정함에 있어 수고해 주신 모든 집필진께 감사의 말씀을 드리며, 향후 이 교재가 심장정지 환자의 진료에 실질적인 도움이 되기를 기대한다.
